How We Use and Disclose Your PHI:

  • Request an Amendment: You may request that we amend or correct your PHI that you believe is inaccurate or incomplete.
  • Receive an Accounting of Disclosures: You have the right to request a list of disclosures of your PHI that we have made, except for certain disclosures.
  • Request Restrictions: You may request restrictions on how your PHI is used or disclosed, though we are not required to agree to these requests.
  • Complaints: You have the right to file a complaint if you believe your privacy rights have been violated.

Changes to this Notice:
We may update this Notice periodically. The effective date of this Notice is May 1st, 2024.

  • HIV-Related Information: We will not disclose HIV-related information except as permitted by State or Federal law.
